In the rods, the cells that work in dim light, vitamin A is built into rhodopsin, the pigment that detects light. MedlinePlus explains that vitamin A “produces the pigments in the retina”, and the NHS says it helps vision in dim light. When you do not get enough, the first sign is usually difficulty seeing in low light, called night blindness.

Vitamin A deficiency is uncommon in the UK, Europe and North America, but it is serious worldwide. The World Health Organization (WHO) calls it the world’s leading preventable cause of childhood blindness and says it is a public health problem in more than half of all countries, especially in Africa and South-East Asia. If you eat a varied diet, you almost certainly get enough, and extra vitamin A will not give you better night vision.

More is not better. The NHS warns that long-term intakes above 1.5 mg a day may affect your bones, and pregnant women should avoid liver and liver products because of their high vitamin A content. MedlinePlus adds that excess vitamin A raises the risk of birth defects and is a particular hazard for children.

Do eye vitamins and supplements work?

For most healthy people, no: eye supplements do not improve eyesight and are not a substitute for a balanced diet. The NEI therefore tells current and former smokers to choose an AREDS2 supplement, which contains no beta-carotene.

Extra lutein and omega-3 added nothing further. In that same trial, neither lutein plus zeaxanthin nor omega-3 fish oil (DHA and EPA) significantly reduced progression to advanced AMD beyond the basic formula.

The zinc in this formula, 80 mg, is far above the 25 mg a day the NHS gives as a general ceiling for zinc supplements without medical advice. That is why an AREDS2 supplement should be started on the advice of an eye specialist, not self-prescribed.

A Cochrane review of antioxidant supplements in AMD (2017) concluded that people with AMD may see some delay in progression, but that the findings rest largely on one major US trial.

Omega-3 for dry eyes: the evidence is mixed. Many people take fish oil capsules for dry eyes. But the DREAM trial in the New England Journal of Medicine (2018) gave 349 people with dry eye disease 3,000 mg a day of fish-derived omega-3 and 186 people an olive-oil placebo for 12 months.
