Meaning of presbyopia in the eye

Presbyopia means that your eyes gradually lose the ability to see things clearly up close. The AAO describes it as a normal part of ageing that people typically begin to notice shortly after the age of 40. It is a refractive error, in the same family as short-sightedness, long-sightedness and astigmatism, but unlike those it is not about the shape of the eye you were born with. It is about the natural lens ageing.

It helps to know the focusing system. When you look at something close, the lens inside the eye gets rounder and more powerful so the image lands sharply on the retina. This automatic adjustment is called accommodation. EyeWiki describes presbyopia as the irreversible loss of this accommodative ability with age. In early life the eye can add roughly 7–10 dioptres of extra focusing power; by about age 50 only around 0.5 dioptre is left, and complete loss typically occurs between about 50 and 55.

Is presbyopia the same as long-sightedness? No, although they feel alike. Long-sightedness (hyperopia) comes from the shape and length of the eye and can be present from childhood; presbyopia comes from the ageing lens and happens to everyone. The NHS lists the stiffening of the lens with age (presbyopia) as one cause of long-sightedness symptoms. The two can also exist together, and a long-sighted person usually notices presbyopia earlier, because the eye was already using some of its focusing reserve to see clearly.

Presbyopia causes: why does near vision fail with age?

The cause of presbyopia is the natural lens becoming harder and less flexible with age. According to the AAO, after about 40 the lens becomes increasingly rigid and cannot change shape as easily as it did in youth. The NEI describes the same process: the lens grows stiffer, so light from close objects cannot be brought into focus on the retina.

Doctors do not fully agree on every detail of how this happens. EyeWiki notes that the exact mechanism is still unknown. Some theories point to changes in the lens itself (hardening and loss of elasticity), others to the ciliary muscle, the tiny fibres that hold the lens, or the tissues around it. For the patient, the practical result is the same.

Three points are worth knowing:

Nothing you did caused it. The AAO is clear that it is a normal ageing process. It cannot be prevented, and there is no way to stop or reverse it.

Starting early can have other causes. EyeWiki lists systemic disease, certain medications, injury and occupations with a lot of close work among the causes of premature presbyopia. If near vision fails clearly before 40, ask for an eye examination to look for the reason.

It does not come from “too much screen time”. Close work makes presbyopia noticeable, but it is the age of the lens that sets the timetable.

Why can laser not simply cure it? Laser surgery reshapes the cornea, the clear window at the front of the eye, but the stiff lens sits behind it. Every laser treatment for presbyopia therefore works around the problem rather than making the lens flexible again.

Glossary: Cornea <use href="#i-arrow"/>

Presbyopia symptoms: the first signs to look for

The classic presbyopia symptoms are blurred close vision, holding things at arm’s length, and tired eyes after reading. The AAO and NEI list the following:

Symptom

What you may notice

Blurred near vision

Menus, labels, phone messages and book print look soft or fuzzy up close

Holding things further away

You move the phone, book or menu out at arm’s length (“my arms are getting too short”)

Eye strain and tired eyes

Aching or sore eyes after reading or close work

Headaches

Especially after close work, as the NEI and AAO both note

Needing more light

Small print is harder to read in dim light, and brighter light makes it easier

Taking glasses off to read

A short-sighted person lifts distance glasses to read, a clue that near focus is failing

Squinting

EyeWiki also lists squinting and working at greater distances as common signs

Symptoms usually creep in over a few years and are often first noticed in dim light, in the evening or when tired.
