The AAO describes it as a cable-like group of fibres that connects the eye to the brain.

The route that a picture takes, step by step:

Retina. Light-sensitive cells turn light into signals, which pass to the retinal ganglion cells.

Optic nerve. The long fibres of the ganglion cells gather at the optic disc, leave the eye, travel behind it through the eye socket and enter the skull through a bony channel.

Optic chiasm. The two optic nerves meet at the base of the brain. Fibres from the inner half of each retina cross to the other side; fibres from the outer half stay on the same side.

Optic tracts and optic radiations. The regrouped fibres continue through a relay station deep in the brain and then fan out towards the back of the head.

Visual cortex. Vision is finally put together at the back of the brain.

Because of this crossing, damage in front of the chiasm usually affects the sight of one eye, while damage at or behind it usually removes the same side of the visual field in both eyes. A visual field test looks for exactly this pattern.

Optic nerve and glaucoma. The nerve is the tissue that glaucoma harms. EyeWiki, the clinical encyclopaedia of the AAO, estimates it affects about 1–5 people in 100,000 a year, mostly aged 20–45, and women about 3 times for every 2 men. The AAO lists autoimmune diseases such as multiple sclerosis (MS) and MOG antibody disease (MOGAD) and viral infections among the causes. Optic neuritis can be the first sign of MS: in the 15-year follow-up of the US Optic Neuritis Treatment Trial, half of the patients developed MS, but the risk was 25% in those with a normal brain scan at the start and 72% in those with one or more lesions on it.

Ischaemic optic neuropathy (poor blood supply)

In ischaemic optic neuropathy, the blood supply to the nerve fails and the nerve is injured. The AAO says it usually causes sudden vision loss and that the loss is usually permanent. The commonest form, non-arteritic anterior ischaemic optic neuropathy (NAION), typically starts at about 57–65 years of age. EyeWiki reports that two-thirds of patients notice it on waking, that it is linked to a small, crowded optic disc and to high blood pressure, diabetes and sleep apnoea, and that about half of patients end up with vision of 20/30 or better. In the other eye, 15–24% develop it within 5 years.

Giant cell arteritis (GCA) is a different cause of ischaemic damage and an emergency. This inflammation of arteries, mainly in people over 50, causes headache, scalp tenderness, jaw pain when chewing and vision loss. The NHS says it needs urgent treatment because it can lead to stroke and blindness. EyeWiki warns that the second eye can be involved within 14 days if it is untreated, which is why steroids are started immediately.

Glaucoma

Glaucoma is a group of eye diseases that damage the optic nerve, most often because pressure inside the eye is too high for that nerve, although it can happen at normal pressure too (NHS, NEI). Risk factors include age over 50, a family history, Black African, Caribbean or Asian ancestry, short-sightedness, long-sightedness and diabetes (NHS). The damage is permanent, but early treatment can often stop it from getting worse.

Papilloedema (swelling from raised pressure in the skull)

Papilloedema means swelling of the optic disc caused by raised pressure around the brain, for example from a tumour, bleeding, a blocked fluid flow or idiopathic intracranial hypertension. Severe cases need hospital treatment.

Pressure from a growth, toxins and vitamin deficiency

The optic nerve can also be squeezed by a tumour or another mass, or poisoned. The AAO’s EyeNet lists vitamin B12 and folate deficiency and some medicines, such as the tuberculosis drug ethambutol, linezolid and isotretinoin, as well as methanol. These typically cause painless, symmetrical blurring with a central blind spot and loss of colour vision, and removing the cause early gives the best chance of recovery.

In June 2025, the European Medicines Agency’s safety committee concluded that NAION is a very rare side effect of semaglutide medicines (Ozempic, Rybelsus and Wegovy), affecting up to 1 in 10,000 people taking them. If you notice sudden or fast-worsening vision on one of these medicines, contact your doctor without delay and do not stop a prescribed medicine on your own.

Inherited conditions and injury

Inherited conditions, such as Leber’s hereditary optic neuropathy, and injury to the head or eye socket can also damage the optic nerve.

Optic atrophy is where untreated damage ends up. EyeWiki defines optic atrophy as the death of the nerve fibres that make up the optic nerve, seen by the doctor as a pale optic disc.
